To start, simply call the office where you would like to begin your treatment. You can click our location link to see a map and get directions.
Here is a handy checklist of things to know before YOUR FIRST VISIT:
To help you save time: Download, print, complete and carefully review all forms regarding our HIPPA guidelines/Privacy Policy; Consent for Treatment form; Personal information form; Medical information and Cancellation/No-show policy. Please bring your medical insurance card, x-rays/MRI information and a list of current medications.
Know your co-pay for our services. We will make every effort to obtain up-to-date insurance benefit information. However, this is not a guarantee of payment. You will need to contact your insurance company to confirm the information given to us is accurate. See list of participating Insurance Plans outlined under FAQ.
Co-payment: We accept: Check, Cash, American Express, Visa and Master credit cards
Note: It is illegal for us as contracted providers with your health insurance to waive co-payments or deductibles. Co-payments are due at the time of each visit. If there is a financial hardship situation, arrangements with our billing office must be made prior to treatment.
Bring your datebook, calendar so that we can schedule future visits.
Arrive 20 minutes early. If you are a new patient we ask that you arrive early to complete the intake paperwork and properly get oriented on our company policies.
Prepare to spend a full hour with us: We will be performing a comprehensive evaluation, which will include treatment and home exercise instructions.
Please make sure to wear comfortable exercise clothing, as well as tennis or athletic shoes. It is best to wear loose-fitting clothes that will enable us to examine and treat you.
